5

Once linked, review your plan details to confirm your coverage effective date, plan type, and enrolled dependents. Download or print your member ID card from the portal.

Activation by Plan Type

The portal you use depends on the type of UHC plan you enrolled in. Use the table below to identify the correct destination for your coverage.

Medical Plans

UHC offers several types of medical coverage, including ACA/Marketplace plans, short-term medical plans, and TriTerm Medical plans. ACA plans are fully compliant with Affordable Care Act requirements and include essential health benefits. Short-term and TriTerm plans offer flexible coverage for temporary gaps and are underwritten by Golden Rule Insurance Company (formerly UnitedHealthOne).

Dental & Vision Plans

Dental and vision plans under activate.uhc.com are underwritten by The Chesapeake Life Insurance Company (formerly SureBridge) and managed at uhcmemberhub.com.

Supplemental Plans

Health ProtectorGuard and other supplemental plans are activated through uhcmemberhub.com. These plans are underwritten by Golden Rule Insurance Company (formerly UnitedHealthOne) and provide extra financial protection alongside your primary coverage.

Student Health Plans

If you are a student enrolled in a university-sponsored UHC plan, activate and manage your plan at uhcsr.com. Student plans are tailored to the academic year and include coverage for on-campus and off-campus care.

After Activation

Once your plan is activated:
  • Your member ID card is available to download immediately in the portal
  • Claims can be submitted and tracked online
  • In-network providers can be searched using the Find a Provider tool
  • Benefit summaries and Explanation of Benefits (EOB) documents are accessible in your account
Activate your plan as soon as you receive your enrollment confirmation — do not wait until you need medical care. Coverage cannot be used until activation is complete.

Personal Representative Access

If you are activating or managing a plan on behalf of another member (such as a dependent or elderly family member), you can register as a personal representative at the personal representative portal. You will need the member’s consent and identifying information to complete the process.
Personal representatives must be authorized by the primary member. Unauthorized access to another member’s account is prohibited and may violate federal privacy laws.
